Her Story

About Angela

Angela Fields has dedicated over 35 years to community health work, drawing inspiration from her mother and early experiences in caregiving roles. As a community health worker, certified doula, and childbirth educator, she founded CHW Concepts LLC in October 2022 after serving in leadership positions at various nonprofits to sustain community health workers on health teams. She holds a double associate's degree in human services and health, supplemented by around 20 stackable credentials and certificates in areas such as health coaching, trained medication aid, end-of-life transition, system navigation, and cancer survivor support and advocacy. Her professional focus includes workforce development, connecting individuals and families to resources, providing health education and navigation, maternal and child health education, chronic disease prevention, mental health services, health equity services, community engagement, and advocacy. Angela is a long-time Minnesota resident who serves on the Department of Human Services Cultural and Ethnic Communities Leadership Council, Health and Democracy Partnership Minnesota, Minnesota Public Health Association board, and as a national community health worker ambassador for Region 5, among other roles. She also leads community initiatives such as Pathways to Wellness, a Blue Cross and Blue Shield of Minnesota-funded health education event, and previously founded Nevaeh Soul over 25 years ago.

01What advice would you give to young women entering your industry?

Find your passion, find your lane, operate within your lane, be open to the journey and the pathways that are ahead, because you never know where they're gonna lead you. Always show up as your authentic self. Listen more than you speak. Listen with intentions, to help the community. And continue to show up. It's definitely rewarding when you see the outcomes, when you see the healthier communities, the healthier individuals, when someone comes back to you and say, I'm where I'm at because of your help.
