Multi-Site Senior Practice Manager
Oak Street Health
Glendale, AZ 85305

Chasity Rogers, MBA, is a healthcare operations executive based in the Greater Phoenix Area with over 10 years of progressive leadership experience across value-based primary care, home health, physical therapy, and healthcare consulting. She currently serves as a Multi-Site Senior Practice Manager at Oak Street Health, where she oversees operations for primary care centers serving Medicare-eligible populations. In this role, she manages cross-functional clinical and non-clinical teams, drives quality improvement initiatives, and ensures strong financial and operational performance across multiple sites with a combined P&L exceeding $8M.

Throughout her career, Chasity has built a strong record of scaling healthcare operations and improving organizational outcomes. Her background includes leadership roles such as Program Director in healthcare consulting—where she generated $2.5M in new revenue—and Branch Operations Manager at BrightStar Care, where she increased revenue by 35% and reduced staff turnover by 40%. She also brings foundational experience in physical therapy operations and patient services management, with a consistent focus on improving care quality, closing care gaps, and strengthening patient engagement in value-based care environments.

Chasity’s leadership philosophy centers on servant leadership, culture-building, and the belief that operational excellence begins with people. She emphasizes recognizing and reinforcing team performance, empowering multidisciplinary staff, and connecting daily operations to patient outcomes rather than metrics alone. In addition to her corporate leadership role, she is Co-Founder and COO of RehabIQ, where she is helping develop an operational framework for industrial athlete rehabilitation in workers’ compensation settings. She holds an MBA and is actively pursuing senior executive opportunities in healthcare operations, including Director and VP-level roles.

Q

What do you attribute your success to?

I attribute my success to a strong belief in creating meaningful impact through others and fostering a culture where small wins are recognized and celebrated, knowing they compound over time. I prioritize purpose, emotional intelligence, and genuine buy-in, understanding that while skills can be taught, intrinsic motivation cannot. As a servant leader, I rely on my team and remain committed to supporting their growth, while approaching challenges with logic, accountability, and the courage to speak up and advocate for what is right.

Q

What’s the best career advice you’ve ever received?

The best career advice I’ve ever received is to always take pride in my work, stand with confidence in what I do, and never hesitate to voice my opinion—because it truly matters.

Q

What advice would you give to young women entering your industry?

My advice to young women entering the industry is to speak up and make your voice heard—your perspective matters. Stay curious, ask questions, and embrace every opportunity to learn, as growth comes from actively seeking knowledge and understanding.

Q

What are the biggest challenges or opportunities in your field right now?

One of the biggest challenges in my field is serving underserved populations who face barriers like mental health needs, housing instability, and limited transportation, all of which impact access to care. In a value-based care model, outcomes directly affect organizational performance, making early intervention, patient engagement, and preventative care critical. This work is also an opportunity to focus on education and support for those who need it most, and it requires genuine compassion and a true commitment to caring for people—something that cannot be taught.

Q

What values are most important to you in your work and personal life?

The values most important to me are servant leadership, integrity, and accountability. I believe in recognizing and reinforcing positive impact, building a strong culture, and empowering others through support and emotional intelligence. While skills can be taught, purpose, buy-in, and genuine care must come from within, and I value a team that is committed to making a real difference rather than viewing the work as just a job.
