Kaley Furner

Licensed Master Social Worker and maternal health advocate with experience in behavioral health, care coordination, and community-based support. Currently supports members at Cityblock Health as a Behavioral Health Community Health Partner, completing screenings (PHQ-9, GAD-7, AUDIT, DAST), coordinating referrals, scheduling, and connecting individuals to mental health/SUD resources through remote, cross-team collaboration. Previously served at Summa Health as a Community Health Worker II/III, managing a caseload of 50+ women, supporting pregnant and postpartum clients, coordinating a food pharmacy, working on multiple government grants, and tracking infant mortality-related data. Also provides freelance birth doula services through DONA International and has additional case management experience with HWS Best Health and social work training/roles at Wellness Grove.
